You may simply call (501) 321-0708 (Hot Springs Office) or (479) 662-4040 (Russellville Office) to speak with one of our intake specialists for individualized attention, fax a verbal order to our office at (501) 321-9567 (Hot Springs Office ) or (479) 964-5653 (Russellville Office) or print out and complete our Referral Form and fax it to one of the fax numbers listed above. We will take care of the rest. In fact, if your patient needs medical equipment, let us know and we will set that up, as well.
